Your Care Journey

Care at 91ÊÓÆµ typically unfolds in three phases: Discovery, Treatment, and Maintenance.

Discovery

The Discovery phase is focused on understanding the full picture behind the pain. This includes reviewing medical history, prior treatments, imaging, symptoms, and the patterns that may help identify pain generators and contributing factors.

For many specialized pathways, such as hypermobility, headache, and autonomic dysfunction, this phase may begin with a comprehensive intake visit with one of our specialized Physician Assistants. This allows our team to map the history, review records, coordinate imaging when needed, and prepare for a highly focused physician consultation.

Patients in certain pathways, including pelvic pain or complex chronic pain, may be scheduled directly with the physician depending on the nature of the concern.

Treatment

Once we have a clearer understanding of what may be driving the pain, we work with each patient to develop a personalized treatment plan. Our approach is not based on a one-size-fits-all algorithm. Instead, each medication trial, diagnostic injection, procedure, therapy recommendation, or referral helps us learn more about the condition and refine the plan over time.

Treatment is often an iterative process. The goal is to identify which interventions are most appropriate, what the body responds to, and how to move forward in a way that is clinically thoughtful and aligned with each patient’s needs.

Maintenance

Once pain generators and contributing factors are better understood, and treatment has been optimized, care shifts toward maintenance. In this phase, the goal is to help patients understand their condition more clearly, recognize patterns, manage flare-ups when possible, and maintain function with the right level of clinical support.

Your Role in the Process

Patients should expect a more detailed and engaged evaluation than they may experience in a standard visit. Complex pain often requires time, careful listening, and active participation from both the clinical team and the patient.

We may ask patients to track symptoms, responses to treatment, medication effects, flare patterns, and functional changes over time. Tracking each distinct pain area or symptom separately can help our team better understand what is improving, what is not changing, and what may need to be reconsidered.

We also host a complementary Pain Support Group on the last Wednesday of every month. Patients and non-patients are welcome to participate

What to Bring to Your First Visit

Please bring the following to the first appointment:

  • Government-issued ID
  • Completed new patient forms
  • All current medications in their original pill bottles, or a complete written medication list
  • Any prior physician notes, procedure records, or relevant medical records
  • Radiology reports and, whenever possible, the actual imaging files on CD or digital format
  • A written timeline of symptoms, including when each distinct pain began
  • A list of prior treatments tried, including what helped, what did not help, and what worsened symptoms
  • Contact information for the primary care provider and other members of the care team

 

Insurance, Billing, and Payment

Because our care is highly individualized and often involves complex, diagnosis-first evaluation, 91ÊÓÆµ is an out-of-network provider. This structure allows us to make clinical decisions based on what each patient needs, rather than limiting care to what may be covered under a standard insurance pathway.

When applicable, our team may verify out-of-network benefits before the visit and provide an estimated patient responsibility. This estimate may include charges, co-payments, co-insurance, deductible amounts, or other patient responsibility required under the health benefit plan.

If the insurance company sends reimbursement directly to the patient for services provided by 91ÊÓÆµ, that payment must be remitted to our office.

For self-pay patients, payment is expected prior to the start of the visit. A non-refundable deposit may be required to secure an appointment.

When financial circumstances make full payment difficult, our team will review available options and may work with patients on a reduced fee or payment arrangement when appropriate.

Appointment Guidelines

To help us keep appointments on schedule and provide each patient with the time and attention their care requires, please review the following guidelines:

  • Please arrive 10 minutes early to complete any remaining paperwork.
  • Expect to spend up to 2 hours with us for the initial visit.
  • All appointments must be scheduled in advance.
  • Walk-ins are not accepted.
  • Current patients may request an urgent visit by phone or email.
  • A $150 fee applies for missed appointments, cancellations with less than 24 hours’ notice, or arrivals more than 20 minutes late.
  • If a patient arrives more than 20 minutes late, the appointment may need to be shortened or rescheduled.

 

Medication Refills and Safety

Please bring all medication bottles or an updated medication list to every appointment. This helps our clinical team maintain an accurate medication record and support patient safety.

Please schedule follow-up appointments in time for any necessary refills. Refill planning is the patient’s responsibility.

Medication refills require an active patient relationship and appropriate follow-up care. A telemedicine visit may be acceptable when clinically appropriate.

Patients receiving medication management must remain actively engaged in follow-up care. We maintain a strict limit of 6 months without an in-person or telemedicine follow-up for any refills.

Urine drug screens may occur at any time to support patient safety and responsible prescribing.

 

Communication With Your Care Team

To provide thoughtful and coordinated care, we may communicate with the referring provider, primary care provider, or other members of the care team when appropriate. Please bring accurate contact information for the clinicians involved in care.

 

Behavior Policy

We are committed to maintaining a respectful and safe environment for patients, staff, and providers. Rude, threatening, or inappropriate behavior may result in discharge from the practice.

Controlled Substance Policy

Controlled substance prescriptions, including opioids, are not provided during the first visit.

Ongoing controlled substance management in isolation is not a service 91ÊÓÆµ offers. When controlled substances are clinically appropriate, they are considered only within the context of a broader treatment plan.
